PREPARE FOR YOUR VISIT

Client intake & consent.

Clear questions and treatment-specific information to support an informed conversation.

Your forms, completed securely.

Aimee uses DocuSign for client intake and treatment consent. Contact Aimee to request the forms for your appointment.

  1. Request your forms

Let Aimee know which appointment you have booked so she can confirm the forms you need.

  2. Use your DocuSign link

When you receive your link, review and complete your forms before your visit. Contact Aimee if you need the link resent or have trouble opening it.

  3. Bring your questions

If anything is unclear, ask before signing. Let Aimee know about changes to your health, medications, skincare or recent treatments.

Why the forms matter

Your intake form helps Aimee understand the full picture before your visit. These details can affect which treatment is appropriate, how it is planned, and the care your skin needs afterward:

  • Medications and supplements. Share what you use, including prescription retinoids, acne medications, and blood thinners, so Aimee can review treatment suitability and timing. Do not stop prescribed medication without your prescriber’s advice.
  • Skin and health history. Mention cold sores, eczema, keloid scarring, recent sun exposure, pregnancy or nursing, and recent procedures so they can be considered in your treatment plan.
  • Allergies and sensitivities. These details help Aimee choose products and discuss possible reactions during and after treatment.
  • Your goals. Knowing what bothers you most keeps the plan focused on results that matter to you.

Your treatment consent form explains the specific procedure, what to expect during recovery, possible side effects, and the aftercare to follow. Read it carefully so you can make an informed decision. Aimee is happy to answer questions before you sign.

Forms questions, answered

Which forms will I receive?

Aimee will confirm the intake and treatment-specific consent forms you need for your appointment. Let her know which treatment you have booked, including corrective care such as Ultra Glo laser, microneedling, or BioRePeel, so she can send the appropriate forms.

When should I complete them?

Before your visit, so appointment time is spent on your skin. If anything in your health or medications changes between signing and your appointment, let Aimee know. Ask about anything you do not understand before signing.

Is my information secure?

Client intake and treatment consent are handled through DocuSign. This website does not collect or store completed forms or health information. Use the DocuSign link Aimee provides to share your intake details.

I already filled out forms. Do I need to again?

Let Aimee know if you have already completed forms, and tell her about any changes to your health or medications. She will confirm whether an updated form is needed before your next visit.
